What is the Convenio Especial?
The Convenio Especial is a scheme that allows certain residents to access Spain’s public healthcare system by paying a monthly fee.
In simple terms, it allows people who are not entitled to public healthcare through employment, self-employment, pension rights, or another route to buy into the Spanish healthcare system.
Once accepted, you receive a Spanish health card and can access healthcare through the Sistema Nacional de Salud (SNS), Spain’s public healthcare system.

Who can apply?
To qualify for the Convenio Especial, you must generally:
Meeting all of the above conditions is essential to be accepted into the Convenio Especial scheme. Each application is assessed on a case-by-case basis by the Social Security (INSS).
Who Does Not Need the Convenio Especial?
You may not need the Convenio Especial if:
- You are employed in Spain.
- You are self-employed and paying into the Spanish Social Security system.
- You are entitled to Spanish public healthcare through another route.
- You receive a state pension and have healthcare rights through an S1 form.
- You have acquired permanent residence rights after five years of legal residence in Spain.
For example, many UK State Pension recipients living in Spain receive healthcare through an S1 certificate and therefore do not need the Convenio Especial.
It is also important to note that the GHIC or EHIC card is only intended for temporary stays and is not a valid long-term healthcare solution for residents in Spain.

What documents are required?
Although requirements can vary slightly between autonomous communities, applicants will generally need:
- Passport, NIE, TIE, or another identity document.
- Evidence of residence for at least the previous year.
- Current padrón certificate.
- Documentation confirming that they do not have healthcare entitlement through another country.
In some cases, applicants may also need a declaration or certificate confirming that they are not exporting healthcare rights from their country of origin.
The Convenio Especial does not replace healthcare entitlement through employment or an S1 certificate. It is intended for people who have no other route into Spain’s public healthcare system.
Where do you apply?
Applications are made through the health service of the autonomous community where you live.
Once approved, you will receive access to Spain’s public healthcare system and a health card that can be used throughout Spain.
